Mae&#8217;n darparu rhaglen 16 wythnos o weithgarwch corfforol i unigolion sy&#8217;n cael eu hatgyfeirio gan weithwyr iechyd proffesiynol y GIG, gan ddefnyddio technegau newid ymddygiad i ymgorffori arferion gweithgarwch corfforol cadarnhaol.<\/p>\n\n\n\n<p>Ar \u00f4l cael eu cyfeirio, gwahoddir cleifion sy&#8217;n bodloni&#8217;r meini prawf i&#8217;w canolfan hamdden leol ar gyfer asesiad cychwynnol gyda gweithiwr proffesiynol atgyfeirio ymarfer corff cymwys. Bydd ganddynt gynnig rhaglen ymarfer corff wedi&#8217;i theilwra, dan oruchwyliaeth am 16 wythnos a bydd eu cynnydd yn cael ei adolygu ar bwyntiau allweddol.<\/p>\n\n\n\n<div style=\"height:15px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<figure class=\"wp-block-image size-large\"><img decoding=\"async\" src=\"https:\/\/www.monlife.co.uk\/wp-content\/uploads\/2023\/03\/Untitled-1-01-1-1024x114.png\" alt=\"\" class=\"wp-image-21432\"\/><\/figure>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Sut y gallaf gyfeirio?<\/strong><\/h3>\n\n\n\n<p>Gellir cael atgyfeiriad gan weithiwr iechyd proffesiynol cydnabyddedig y GIG (e.e. \u2013 meddyg teulu; Nyrs; Ffisiotherapydd neu Ddietegydd) trwy&#8217;r system newydd ar y we, o&#8217;r enw Porth Theseus. Gall Gweithwyr Iechyd Proffesiynol syn atgyfeirio ddarganfod mwy am Borth Theseus yma: <a href=\"https:\/\/icc.gig.cymru\/gwasanaethau-a-thimau\/cynllun-atgyfeirio-cleifion-i-wneud-ymarfer-corff-cymru\/system-rheoli-cleifion-newydd\/\">System Rheoli Cleifion Newydd &#8211; Iechyd Cyhoeddus Cymru<\/a>.<\/p>\n\n\n\n<p>Ar \u00f4l cael eich atgyfeirio, byddwch yn cael eich cofrestru ar gyfer asesiad cyntaf gyda Gweithiwr Proffesiynol Atgyfeirio Cleifion i Wneud Ymarfer Corff hyfforddedig.
